肝门胆管癌的手术治疗

Surgical resection for hepatic hilar cholangiocarcinoma.

目的总结肝门胆管癌的手术治疗的经验。方法回顾性分析本院1997年1月～2006年1月9年肝门胆管癌手术切除25例患者的临床资料和随访结果。结果25例手术切除患者中行根治性切除（R0）11例,非根治性切除14例（R1,R2）,根治性切除组中位生存期32个月,1、3、5年生存率分别为91%、36%、12%,好于非根治性切除组（P〈0.05）。结论手术切除是治疗肝门胆管癌的有效治疗方法,根治性切除能更好地延长患者生存期。

Objective To summarize the experience in surgical resection for hepatic hilar cholangiocarcinoma.Methods The clinical and follow-up data of 25 patients with hepatic hilar cholangiocarcinoma receiving surgical resection in our hospital between January 1997 and January 2006 the 9 year were retrospectively analyzed.Results Of the 25 patients,11 underwent radical resection and 14 palliative resection.In the radical resection group,the median survival time was 32 months and the 1、3 and 5 year survival rates were 91%、36%、12%,they were significantly higher than those in the palliative resection group（P〈0.05）.Conclusion Surgical resection is the most effective method for treatment of hepatic hilar cholangiocarcinoma,the radical resection can prolong and improve the survival time of the patient.
