腺苷蛋氨酸治疗妊娠肝内胆汁淤积症的临床剂量研究

Different dose of S-adenocyl-L-methionine for intrahepatic cholestasis of pregnancy.

目的研究不同剂量的腺苷蛋氨酸治疗妊娠肝内胆汁淤积症(ICP)的临床疗效。方法将60例ICP患者分为两组,每组各30例,观察组30例采用腺苷蛋氨酸2g/d静脉滴注治疗,对照组30例采用腺苷蛋氨酸1g/d静脉滴注治疗,治疗2周后比较肝功生化指标、瘙痒症状的改善情况。并且比较分娩后两组围生儿预后及产妇预后情况。结果观察组TBA、ALT、AST、DBIL下降较对照组快,两者差异有统计学意义(P<0.05)。并且观察组对孕周延长、新生儿体重、降低产时缺氧率改善效果明显。结论2g/d腺苷蛋氨酸及1g/d腺苷蛋氨酸均能有效的缓解ICP症状,但2g/d腺苷蛋氨酸疗效更好,有利于提高围生儿质量及产妇预后。

Objective To evaluate the effection of the different dose of S-adenocyl-L-methionine for intrahepatic cholestasis of pregnancy（ICP）.Methods Sixty patients with intrahepatic cholestasis of pregnancy were randomized to take a dose of 2g/d or 1g/d of S-adenocyl-L-methionine,for 2 weeks.Then compare the maternal blood level of biochemical parameters,titillation,prognosis of the perinatal fetus and maternal.Results Compared with control group,the ALT,AST,TBA,TBIL,DBIL were significant decreased after treatment of 2g/d（P〈0.05）.The pregnancy period was significant prolonged;the weight of newborns were increased,the fetal distress were decreased.Conclusion 2g/d of S-adenocyl-L-methionine is more effective than the dosage of 1g/d for intrahepatic cholestasis of pregnancy.
