足月妊娠不同规格水囊引产促宫颈成熟的疗效分析

Analysis of the Effect of Maturing Cervix by Induced Labor with Different Sizes of Water Bags When Pregnancy at Term

【目的】评价目前常用的足月妊娠不同规格水囊引产促宫颈成熟方法的效果,为足月妊娠促宫颈成熟和引产提供有效、安全、方便同时为广大孕妇接受的水囊规格引产方法。【方法】选择有引产指征的足月妊娠产妇255例,随机分为4组:大水囊组52例;中水囊组73例;小水囊组80例;超小水囊组50例,比较4组产妇的宫颈评分、分娩结局、新生儿结局。【结果】各种水囊规格促宫颈成熟效果,成功率,分娩方式,引产时间以及各组新生儿窒息发生率和胎儿窘迫发生率差异无显著性。【结论】低位水囊有显著的促宫颈成熟的作用,配伍缩宫素在足月妊娠促宫颈成熟和引产过程中有效,但在其有效、安全、方便性上,不同规格的水囊引产无统计学意义。因超小水囊引产操作方便,用Foley导尿管即可,故建议应用超小水囊促宫颈成熟治疗。

[Objective]To evaluate the effect of maturing cervix by induced labor with different sizes of water bags when pregnancy at term. [Methods]Two hundred and fifty five pregnancy women who had indications of induced labor were divide into four groups randomly： Group A （big water bag, n =52）, Group B （middle sized water bag, n =73）, Group C （small water bag, n =80） and Group D （tiny water bag, n =50）. Cervix maturity, pregnancy outcome and newborn status were compared among the four groups. [Results]There was no significant difference in the effect of maturing cervix, successful delivery, delivery style, the time of in- duced labor, the incidence of neonatal asphyxia and fetal distress among different groups. [Conclusion]Water bag with lower position has a significant effect for maturing cervix, and combined oxytocin is more effective during maturing cervix and induced labor at term. However, there is no statistical significance in effectiveness, safety and convenience among different sizes of water bags. Foley catheter can be easily used as tiny water bag labor, thus tiny water bag is recommended to be applied in maturing cervix.
