^1H-MRS study the in patients with the major depression

摘要 目的:探讨抑郁症患者1H-MRS的表现。方法:对33例抑郁症患者及18例健康对照组行MRI、1H-MRS扫描,兴趣区为双侧海马,双侧扣带回前部(anterior cingulate cortex,ACC),双侧背外侧前额叶皮层(dorsolateral prefrontal cortex,DLPFC)。结果:抑郁症患者显示脑沟、脑池增宽,脑内未见异常信号灶;双侧海马NAA/Cr明显减低,与对照组相比有显著性差异(P<0.01),双侧背外侧前额叶皮层NAA/Cr明显减低,与对照组相比,有显著性差异(L:P<0.01,R:P<0.01);抑郁症患者背外侧前额叶皮层Cho/Cr比对照组明显增高(L:P<0.01,R:P<0.01)。结论:1H-MRS能够揭示抑郁症时海马及背外侧前额叶皮层存在着神经元的丢失和细胞膜磷脂代谢的异常。 Objective:To study the manifestations of ^1 H-Magnetic resonance spectroscopy(^1H-MRS) in patients with major depressive disorder(MDD).Methods:33 patients with MDD and 18 volunteers similar in age and gender to the patients were scanned on a 1.5 T magnetic resonance scanner.Voxels were placed bilaterally in the dorsolateral prefrontal cortex(DLPFC),the anterior cingulate cortex(ACC) and the hippocampus.Of the NAA/Cr,Cho/Cr and MI/Cr were obtained.Results:No signal abnormalities were noted qualitatively on MRI scan of the patients or control group.Using 2-dimensional CSI^ 1 H-MRS,bilateral hippocampus NAA/Cr ratio was lower in MDD patients compared with healthy control subjects(P〈0.01).DLPFC NAA/Cr ratios were lower on the left hemisphere(P〈0.01) and the right hemisphere(P〈0.01) in depressive patients compared with healthy control subjects.DLPFC Cho/Cr ratios were higher on the left hemisphere(P〈0.01) and the right(P〈0.01) in patients with MDD compared with control subjects.Conclusion:The findings support the existence of the reduced neuronal viability or function in the hippocampus and DLPFC and altered membrane phospholipids metabolism in the DLPFC for patients with MDD.^1 H-MRS can provide metabolic and biochemical changes of the brain in vivo.
