冠心病介入治疗的舒适护理

COMFORTABLE NURSING FOR CORONARY HEART DISEASE PATIENTS UNDERGOING INTERVENTION THERAPY

目的观察舒适护理模式对冠心病介入治疗后患者不适症状及并发症的影响。方法将120例行冠心病介入治疗患者随机分为实验组和对照组,每组各60例,对照组采用两条弹性绷带交叉粘贴包扎股动脉,术后实行常规平卧位卧床24h,术侧下肢制动12h,给予一般常规护理。实验组采用改良弹力绷带包扎法,即在对照组的基础上增加第三条弹性绷带包扎,第三条弹力绷带在两条绷带的交叉点上横跨粘贴包扎,术后3h后,即可在床上平行移动身体,在保持术侧肢体伸直的前提下,用软枕帮助患者术侧侧卧,与平卧位交替,并做好心理护理。结果对两组患者舒适情况比较(包括焦虑、烦躁、排尿困难、腰酸背痛、失眠),差异有统计学意义,(均p<0.05)。结论舒适护理缩短患者绝对卧床和术侧肢体制动的时间,减少了术后不适和并发症,增加患者舒适度。

Objective To investigate the effect of comfortable nursing on uncomfortable symptoms and complication in patients with coronary heart disease after interventional therapy. Methods One hundred and twenty cases were divided randomly into the experiment group and the control group with 60 cases in each group. Two elastic crossed bandaging was used in the control group, the patients were allowed to rollover conventionally 24 h after operation, braking side of the lower limb 12 h, taking the general routine care. A new pressurized bandaging with three elastic bandages on the base of the method of control group was used in the experiment group. 3h after operation, the patients can move their body parallel in bed ; under the condition of maintaining the extension of operated limb, help patient to lie on back and on the operated side alternately with a soft pillow, and make a mental care. Results There was statistical difference in patients＇comfort feeling （Include anxiety, irritability, difficulty urinating, notalgia, insonmia） between the experiment group and the control group （p 〈 0.05）. Conclusion Comfortable nursing in the experiment group decreased the time for patients in forced attitude on bed, reduced the discomfort after operation and decreased the workload of nursing and improved the nursing quality.
