一孔法腹腔镜与开腹阑尾切除术的比较研究

Comparison Study on Laparoscopic Appendectomy with One Trocar and Open Appendectomy

目的探讨一孔法腹腔镜阑尾切除术的应用价值。方法回顾性分析4年来我院阑尾切除术263例的临床资料。行LA的127例与常规开腹手术136例进行对比。比较两组手术时间、住院时间、术后排气时间、住院费用及并发症等。结果LA组手术平均时间,术后住院时间,术后排气时间较OA组明显缩短(P<0.01)。住院费用无明显差异(P>0.05)。LA组无并发症。OA组切口感染6例。结论一孔法LA较OA创伤轻,康复快,住院时间短,并发症少,但住院费用无明显差异,值得临床推广应用。

Objective To study the effectiveness of laparoscopic appendectory with one trocar. Methods The clinical data of 263 cases of appondectory in recent 4 years in our hospital were reviewed retrospectively.127 cases underwent LA. Which was compared with 136 cases who under went OA in hospitalization expenses,operative times. The day of hospitalization and post-perative complication. Results Operative time,day of postoperative hospitalization and passage of gas by anus time in LA was shorter than those in OA P 〈 0.01 ）. There was no significant different betweent LA and OA in hospitalition expenses P 〉 0.05）. None had postoperative complication in LA, but 6 cases were effected in 0A. Conclusion The result demonstrate that LA has many advantages compare with OA,such as less trauma,shorter rehabilition peried, day of hospitalization is shorter. But hospitalition exponesses not higher than OA. So it can be use extensively.
