A1级妊娠期糖尿病分娩时机探讨

A Study of the Laboring day in for Pregnancy with A1 Gestational Diabetes Mellitus

目的比较不同孕周A1级妊娠期糖尿病(gestational diabetes mellitus,GDM)母儿的并发症情况,探讨A1级妊娠期糖尿病的分娩时机。方法分析2005年7月-2007年6月在本院分娩的131例A1级GDM患者的临床资料,比较不同孕周分娩的孕妇及其围产儿的结局。结果各不同孕周组A1级GDM孕妇的年龄、身高、体质量差别均无统计学意义(P>0.05);各孕周组孕妇及其围产儿的并发症并未随孕周增长而增加,除新生儿重度窒息外(P<0.05)其余项目在各组间差别均无统计学意义(P>0.05)。A1级GDM羊水过少16.79%,胎膜早破24.43%,明显高于分娩人群。结论White分类A1级的GDM孕妇在40周引产是安全的;A1级GDM孕妇合并羊水过少和胎膜早破明显增加。

Objective To discuss the applicable delivery day and the related complications in the period of termination of pregnancy with A1 gestational diabetes mellitus（GDM）. Methods 131 eases of pregnant women with A1 GDM that delivered in our hospital during July 2005- June 2007 were analyzed retrospectively. Results There were no difference between 5 groups on age, height,weight, maternal and neonatal outeome（P〉0.05） except of the severe asphyxia （P〈0.05）. The incidence of oligohydramnios（16.79%） and premature rupture of fetal membrane（24.43%） were higher than those observed in common pregnancy. Conclusions 1. Women with A1 GDM can deiliver after 40 weeks＇pregnancy; 2. The percentages of oligohydramnios and premature rupture of fetal membranes in GDM are higher in A1 GDM than those in common pregnancy.
