HGF、CXCR-4、VEGF-D表达与甲状腺乳头状癌颈淋巴结转移的关系 被引量:6

Relation between expression of HGF,CXCR-4,VEGF-D and the lymph node metastasis in papillary thyroid carcinoma (PTC)

摘要 目的:通过检测HGF、CXCR-4、VEGF-D蛋白在甲状腺乳头状癌(papillary thyroid carcinoma,PTC)中的表达差异,探讨三种因子与PTC淋巴结转移的关系。方法:随机选取有完整病例记录和追踪观察的乳头状癌病例70例,采用免疫组化ABC法,研究HGF、CXCR-4、VEGF-D等基因产物在PTC的表达情况。结果:70例PTC病人中,三种因子在有淋巴结转移的阳性表达率分别为91.30%、71.74%和89.13%,三种因子在无淋巴结转移组的阳性表达率分别为66.67%、37.50%和37.50%,有淋巴结转移组的阳性表达率均高于无淋巴结转移组,差异有统计学意义;在70例PTC病人中,VEGF-D和CXCR-4表达均为阳性的有39例,二者在PTC中的表达有显著相关性,(相关系数r=0.502,P<0.05)。结论:HGF、CXCR-4可能影响PTC癌细胞的迁移能力而引起淋巴结转移;VEGF-D通过诱导淋巴管内皮细胞新生和淋巴管生成,从而促进PTC淋巴结转移;VEGF-D促进PTC淋巴结转移的作用可能需趋化因子CXCR-4协同参与。 Objective : To study the expression level of 3 factors ( CXCR - 4, HGF, VEGF - D) and the correlation between the 3 factors and lymph node metastasis in PTC. Methods: Total of 70 cases of PTC with complete clinical records and preliminary diagnosis were picked out from patients in Tianjin Cancer Hospital. The expression of HGF, CXCR - 4, VEGF - D in 70 cases of PTC were assayed by using immunohistochemical staining. Results: In 70 cases of PTC,the rate of immuno - positive staining HGF CXCR -4 VEGF - D were 91.30% ,71.74% ,89. 13% with lymph node metastasis and 66.67%, 37.50%, 37.50% without metastasis respectively, ( P 〈 0.05 ) ; The rates of simultaneous immuno - positive staining of VEGF - D and CXCR -4 in 70 papillary thyroid carcinoma were 55.71% (39/70). The positive relationship has been found between the expression status of VEGF - D and CXCR - 4 and cervical lymph node metastasis. Conclusion: HGF and CXCR-4 might be useful to promote lymph node metastasis of PTC by affecting the migration ability of carcinoma ; VEGF - D might cause lymph node metastasis of PTC by lymphangiogenesis ;The expression of VEGF- D is correlated with the expression of CXCR- 4 in PTC, the patients with VEGF - D positive and CXCR -4 positive had high possibility of lymph node metastasis.
