HBeAg与IL-18表达的关系对轻度慢性乙肝的影响及其临床意义

EFFECT OF THE RELATION OF THE INTERLEUKIN-18 EXPRESSION AND HBEAG STATUS ON PATIENTS WITH LOW GRADE CHRONIC HEPATITIS B AND ITS CLINICAL SIGNIFICANCE

[目的]探讨HBeAg状态与IL-18表达的关系及其对轻度CHB患者的免疫应答功能、肝脏炎症程度的影响。[方法]检测轻度CHB患者血液中IL-18、HBVDNA、HBeAg、β2M、ALT等并作出统计分析。[结果]在轻度CHB患者中,HBeAg阴性患者IL-18、ALT水平明显升高(P﹤0.01,P﹤0.05),2者呈明显正相关(r=0.574、P﹤0.05),IL-18异常率较ALT异常率更高(χ2=6.08,P﹤0.05)。HBeAg阳性患者IL-18水平正常,IL-18异常率与ALT异常率比较无统计学差异(χ2=2.487,P﹥0.05)。HBeAg阴性患者HBV-DNA拷贝量明显低于HBeAg阳性患者(P﹤0.01)。[结论]HBeAg与IL-18表达存在一定内在联系,且对宿主的免疫应答功能及免疫调节发挥了重要作用。血清IL-18可以作为评价HBeAg阴性CHB患者的肝脏炎症程度的血清学指标。

[Objective] To explore the relation between HBeAg status and expression of Interleukin-18 and its impact on the function of immune response and inflammation degree of liver among the patients with low grade chronic hepatitis B. [Methods] The IL-18, HBV-DNA, HBeAg, β2M and ALT etc. of low grade CHB patients were detected and analyzed. [Results] Among these low grad chronic hepatitis B patients, IL-18 and ALT of the patients with HBeAg （-） were significantly higher than the controls （P 〈 0.01, P 〈 0.05）, and IL-18 showed significantly positive correlation with ALT （r = 0.574, P 〈0.05）, and abnormality rate of IL-18 was significantly higher than thai of ALT （X^2 = 6.08, P〈0.05）. IL-18 of patients with HBeAg （＋） group showed to be nonual, and the abnomlality rate of IL-18 had no signifieandy difference when compared with the rate of ALT （X^2 = 2.487. P 〉 0.05）. bul copy number of HBV DNA among patients with HBeAg （＋） group was significandy higher than that of patients with HBeAg （-）. [ Conclusion] There was internal relationshio between HBeAg and expression of Inter-leukin-18, which had great impact on the immune response and immunological regulation. IL-18 in serum could be regarded as an effectively serology index to evaluate the severity degree of liver inflammation among CHB patients with HBeAg （-）.
