摘要
目的:比较新生儿听力普遍筛查方案两个不同的初筛时间的通过率和转诊率,探讨首次听力筛查时间与成本效益的关系。方法:该院出生新生儿分为两组:出生3日筛查组(A)和满月筛查组(B)。分别与出生后3天和满月体检时进行畸变产物耳声发射(DPOAE)听力筛查。未通过(refer)者,1个月后进行复筛。复筛阳性者转诊至听力评估中心。结果:A组2305例,初筛通过率82%,复筛通过率85%;B组初筛通过率94%,复筛通过率63%(P<0.001)。两组的转诊率分别为1.65%和1.85%(P>0.05)。6例(1.6‰)最后确诊为听力损失。结论:满月时进行听力筛查可提高初筛通过率,从而减少复筛人次,节约筛查成本。
Objective: To compare the pass rates and refer rates of two primary hearing screening times and explore more economical schedule. Methods: The newborns born in our hospital were divided into two groups: group A and group B. Hearing screened by DPOAE ( distortion product otoacoustic emission ) on 3 day after birth in group A; In group B hearing screened on 1 month old. The babies no - passcd in the first screening were screened secondly one month later. The babies positive were referred to auditory assessment center for diagnosis. Results: Group A (n =2 305) had pass rate 82% at primary screening and 85% at secondary screening; Group B ( n = 1 348) had pass rate 94% ( P 〈 0. 001 ) and 63 % ( P 〈 0. 001 ), respectively. Refer rate of group A was 1.65 %, group B was 1.85 %. ( P = 0. 932 ) . Six of the infants (1.6‰) were diagnosed as hearing loss. Conclusion: Hearing screening at one month old of infant has higher pass rates than at three day old. It may be helpful for reducing the cost of neonatal hearing universal screening.
