胸外伤合并急性呼吸窘迫综合症诊治体会

Study of the treatment effect of chest wound concurrent with acute respiratory distress

目的总结胸外伤合并急性呼吸窘迫综合症(ARDS)的诊断及治疗经验。方法回顾性分析我院1996年3月至2007年3月10例胸外伤合并ARDS的临床资料。结果:绝大部分病例均治愈出院。结论胸外伤并发ARDS是多脏器功能衰竭的前奏及主要的死亡原因之一,积极治疗ARDS可大大降低MODS的发生,降低病死率。早期诊断,正确掌握机械通气指征,采用保护性机械通气,及时治疗原发病和处理合并征,是治疗ARDS有效的治疗方法。

Objective To summarize the experience in the diagnosis and treatment of chest wound accompanied by acute respiratory distress syndrome （ARDS）.Methods Clinical data of 10 ARDS patients admitted in our hospital between March of 1996 and March of 2007 were retrospectively analyzed.Results Most of the studied cases had recovery and were discharged from the hospital after treatment.Conclusion Chest wound concurrent ARDS may lead to multi-organ failure,which can result in death.An active treatment of the disease can greatly reduce the possibility of having MODS,and reduce the mortality rate.The effective treatment of ARDS includes an early diagnosis,proper handle the way of mechanical ventilation,adopt protective mechanical ventilation,timely treatment of primary disease and its complications.
