注射器不同位置放置时胰岛素微量注射过程中的浓度测定

摘要 目的 探讨注射器不同位置放置时微量注射胰岛素(Regular Insulin,RI)稀释液过程中的浓度变化,以指导三升袋输注过程中合理使用RI。方法 20ml注射器抽取RI稀释液20ml,其中含19.5ml的5%葡萄糖,0.5ml的RI20U。根据注射器水平和垂直放置以及乳头位置的不同将试验分为3组,泵速4U/h,分别于注射前、注射后1h、2h、3h、4h分别取样1ml,使用电化学发光分析仪测定RI浓度。结果 微量注射泵在3种位置5个时间段注射RI溶液的浓度变化值为:33.3±2.7mU/L,35.0±3.5mU/L,38.3±4.6mU/L,38.4±3.6mU/L,36.1±3.9mU/L,5组之间方差分析无统计学差异(P〉0.05)。结论 用微量注射泵注射RI稀释液的过程中浓度较稳定,配合3L袋输液能够减少RI浓度变化对于血糖的影响。 Objective To guide the use of insulin in infusing three litre bag the concentration change of diluted insulin was studied with miero injeetion pump and injeetors plaeed in different positions. Methods 20 ml diluted insulin was taken out with 20ml injeetor, whieh eontained 19.5 ml 5% glueose and 0. 5 ml insulin 20 U. Three groups were divided aeeording to the injeetor's horizontal or upright positions and nipple's positions. 1 ml sample was collected respectively before injection and every 1 , 2 , 3 , 4 h after injection with injection speed of 4 U/h. The eoneentration of insulin was measured with eleetro -chemistry irradianey assay instrument. Results The eoneentration of diluted insulin during five different periods when injected with miero injection pump and placed in three kinds of different positions were 33. 3 ± 2. 7, 35.0 ± 3. 5, 38. 3 ± 4. 6, 38. 4 ± 3.6, 36. 1 ± 3.9 mU/L dividedly, whieh had no signifieaut differenee among five groups (P 〉 0. 05) . Conclusion The eoneentration of diluted insulin when injected with miero injection pump is relatively stabile, whieh eould reduee the change of blood glucose induced by ehange of insulin eoneentration in infusing three litre bag.
