颊粘膜肌瓣单独或联合颊脂垫修复舌腭缺损

USE OF BUCCAL MUSCULOMUCOSAL FLAP WITH OR WITHOUT BUCCAL FAT PAD FOR REPAIR OF TONGUE OR PALATE DEFECTS

应用颊粘膜瓣单独或联合颊脂垫修复舌腭缺损是一种简单而有效的手术方法。笔者自１９３９年至１９９６年，应用该方法修复舌癌切除后≤１／２舌缺损１０例，修复软腭缺损２例。瓣全部成活，吞咽、语言功能恢复快，外形满意。术后几天开口约１．５ＣＭ～２．０ｃｍ，２周后张口逐渐恢复正常。本技术为舌半侧以内缺损和软腭缺损的修复提供了可靠实用的新方法。

The use of buccal musculomucosal flap(BMMF)with or without buccal fat pad(BFP)as a graft for tongue or palate defect repair is an alternative for the surgeon,yet it is a simple and effective. From 1993-1996,we have used BMMF with or without with BFP as a pedicled graft for closure of tongue or palate defect for 12 patients.10 of them were operated on for SSC of tongue and a defect of 1/2 or < 1/2 tongue and a part of alveolar proecess formed.A 2 cm-wide BMMF was raised backwards from oral commissure to retromolar trigene with a full layer of buccinator muscle still attached in order to ensure blood circulation to the flap.After raising BMMF,the BFP was pulled out and put it on the dead space created by cutting down the alveolar process.We then cover the raw surface of the tongue defect by rotating the BMMF downwards.2 patients were operated on for soft palate malignant tumor,the oronasal fistula created.Both sides BFP ane BMMF were used to cover the fistala.All the BMMFS were survival.The shapes of the tongue and the soft palate were very good.The function of speech and swallow recovered quickly from the 2nd day after operation.There were no salivation and difficulty in mouth opening.
