临床应用抗菌药物合理性调查评价

Evaluation on rational application of antimicrobial agents in clinic

目的了解某院抗菌药物临床应用中存在的问题。方法依据《抗菌药物临床应用指导原则》设计调查表,制定合理应用抗菌药物评价指标;采用回顾性调查方法,每季度集中进行一次抗菌药物临床应用合理性调查评价。结果共调查858例患者,应用抗菌药物者718例(83.68%),其中治疗用药261例(36.35%),预防用药457例(63.65%);手术科室抗菌药物使用率为93.92%,非手术科室为70.87%。手术科室预防用药占82.14%。治疗用药患者中病原学送检率为26.05%。结论该院抗菌药物使用率高,手术感染预防用药不规范,病原学送检率低,是抗菌药物不合理应用的主要表现。

Objective To investigate the problems in the clinical application of antimicrobial agents in a hospital. Methods Evaluation standards of the rational application of antimicrobial agents was made according to ＂Guidelines for clinical application of antimicrobial agents＂; Retrospective investigation on rational use of antimicrobial agents in clinic was conducted once in a season. Results Eight hundred and fifty-eight patients were surveyed, 718（83.68 %） patients received antimicrobial agents, 261 （36.35%） and 457（63.65%） were for treatment and prophylactic application respectively; the usage rate of antimicrobial agents in patients in operating room and non-operating room was 93. 92% and 70. 87% respectively. Prophylactic application in patients in operating room was 82. 14%. Pathogenic test for samples from patients receiving treatment antimicrobial agents was 26.05%. Conclusion The usage rate of antimicrobial agents in this hospital is high, the main problems are irrational prophylactic application of anti-microbial agents in operative infection, pathogenic test rate for the sample is low.
