早期应用胰岛素与胰岛β细胞功能的关系

摘要 目的:观察早期应用胰岛素治疗对新诊断的2型糖尿病(T2DM)患者胰岛β细胞功能的影响。方法:44例初次诊断的2型糖尿病患者,均接受口服葡萄糖耐量试验(OGTT),测定胰岛素治疗前后患者的血清胰岛素和C肽水平,并计算胰岛β细胞功能指数(HOMA-β)和胰岛素抵抗指数(HOMA-IR)。结果:胰岛素治疗后本组患者的胰岛功能测定显示0、30、60、120和180 min的血清胰岛素和C肽均较治疗前明显升高,HOMA-β明显升高,HOMA-IR有所降低,差别有统计学意义;而体质指数(BMI)有所升高,但差别无统计学意义。结论:早期应用胰岛素治疗,可使胰岛素分泌减低的初次诊断2型糖尿病患者胰岛β细胞得到休息,尽早恢复功能。 OBJECTIVE To observe the effect of insulin short - term early treatment on the pancreatic B - cell function in new type 2 diabetic mellitus ( T2DM ) patients. METHODS A total of 44 Han people were involved in the study, who are new type 2 diabetic mellitus. All of them accept the oral glucose tolerance test. Insulin and C - peptide serum levels by chemil were measured before and after insulin treatment. Then calculate pancreatic β - cell function index and insulin resistance index. RESULTS The levels of insulin and C - peptide were increased at 0,30,60,120,180 min in OGTT in T2DM after insulin treatment, HOMA - β obviously step up. HOMA -IR cut down, there was statistics difference. Body mass index is heighten little ,but differ-ence is not statistically significant. CONCLUSION Shorttime early insulin treatment can partially improve the β -cell function of T2DM with lower function and made it rest, restoration of function.
