米力农治疗慢性肺原性心脏病临床疗效观察

Clinical Curative Effect of Milrinone on Patients with Chronic Pulmonary Heart Disease

目的观察米力农治疗慢性肺原性心脏病的疗效。方法将60例慢性肺原性心脏病患者,随机分为对照组和治疗组。所有患者在吸氧、抗感染、解痉、改善通气、纠正酸碱失衡、电解质紊乱等综合治疗的同时,治疗组给予米力农注射液,具体方法:米力农5mg溶于0.9%氯化钠注射液100ml中,静脉滴注,每日1次,7天为1疗程。对照组在综合治疗的同时,给予利尿、洋地黄类强心剂治疗。观察治疗前后症状、体征、血压、心率及心功能指标。结果米力农治疗组的心功能改善疗效明显高于对照组。结论米力农治疗慢性肺原性心脏病疗效可靠,慢性肺心病心衰应用洋地黄类强心剂疗效较差,易导致中毒,选择米力农既可发挥正性肌力作用,又可扩张血管、减轻心脏负荷,疗效肯定、安全。

Objective To observe the efficacy of Milrinone in the treatment of chronic pulmonary heart disease. Methods 60 cases with chronic pulmonary heart disease were randomly divided into control group and treatment group, the former was treated with the comprehensive treatment of oxygen inhalation,anti-infection, spasmolysis,ventilation improvement and acid and alkali unbalanced electrolyte disorder correction and the latter was together with the Milrinone infusion 5mg dissolved in 0.9 % sodium Chloride 100ml for once a day, seven days for a treatment course. During the comprehensive treatment period, the control group was also treated with the diuretic, the digitalis-class cardiotonic. The symptom, blood pressure, heart rate and indexes of cardiac function were observed during the treatment period. Results Compared with the control group, Milrinone significantly improved the treatment group＇s cardiac function. Conclusion Milrinone is an effective and safe drug,which can exhibite positive inotropic effect, expand the blood vessel and reduce the cardiac loading in patients with chronic pulmonary heart disease. The clinical effect of digitalis-class cardiotonic treatment is bad and easy to cause poison.
