摘要
目的探讨维吾尔族老年不稳定心绞痛(UA)患者与高敏C反应蛋白(hs-CRP)水平及颈动脉超声检查变化及其相关性。方法将109例观察对象中56例不稳定心绞痛(UA)患者作为病例组,对照组53例。应用酶联免疫吸附法(ELISA)检测血清hs-CRP水平。多普勒超声检查比较两组颈动脉内-中膜厚度(IMT)及斑块发生率,对比研究hs-CRP与IMT的相关关系。结果病例组在hs-CRP、IMT、斑块发生率上较对照组差异有统计学意义(P<0.01);将109例研究对象以血清hs-CRP 3mg/L为界,分为hs-CRP≤3mg/L组(66例)和hs-CRP>3mg/L组(43例),结果两组患者IMT、斑块发生率和不稳定心绞痛发生率间差异均有统计学意义(P<0.05);经Pearson积差相关分析,颈动脉IMT与血清hs-CRP水平呈正相关(r=0.434,P<0.01),二者间呈正相关关系。结论血清hs-CRP水平可作为反映不稳定心绞痛患者的敏感标志物,颈动脉粥样硬化程度可以反映不稳定心绞痛的斑块不稳定性,炎症反应可能参与颈动脉硬化形成。
Objective To explore the relationship among the high senstive C - reactive protein ( hs - CRP), carotid artery atherosclersis and the uigur aged patients with unstable angina pectoris (UP). Methods 56 patiens with unstable angina pectoris were UP group, the other 53 without coronary heart diaease were control gruup. The levels of hsCRP was measured by means of Enzyme-Linked- Immonosorbent- Assay (ELISA). Compared with the cartid intima- medio thinkness (IMT) and carotid plague score in two groups patiens by ultrasound. Their results were analyzed by correlate analysis. Results The levels of the hsCRP were significantly higher in UP group than in contral group (4. 72 ±2. 55mg/1 vs 1.85 ±0. 79mg/l). There were significant difference in intimin - media thickness ( 1.82 ± 0. 66mm vs 1.01 ± 0. 40mm) and the prevelence rate of atherosclerotic (85. 7%vs35. 8% ) between the two groups. The thickness of IMT in the subjects with hs - CRP 〉 3mg/L (n =43) were 1.80 ± 0. 63mm, the prevelence rate of atherosclerotic were 83.7%, the prevelence rate of the unstable angina pectoris were 83.7% ; while the thickness of IMT with hs - CRP≤3mg/L ( n = 66) were 1.17 ±0. 59mm, the prevelence rate of atherosclerotic were 83.7%, the prevelence rate of the unstable angina pectoris were 30. 3% ; there were significantly different between in two groups. The levels of the hsCRP were correlated positively with the IMT ( r = 0. 434, P 〈 0. 000). Conclusion The levels of the hsCRP was sensitive predictors of the UP, The condition of carotid artery atherosclerosis may reflect the condition of the unstable angina pectoris. Inflammatory response might play an important role in the carotid artery atherosclerosis.
