摘要
目的:探讨宫内节育器(IUD)异位的病因及预防、护理方法。方法:对13例宫内节育器异位患者的病历资料进行回顾性分析。结果:本组腹腔镜手术取环3例,成功2例,失败1例,改行开腹手术取环成功;宫腔镜手术取环3例,失败后均改行开腹手术取环成功;直接开腹手术取环6例均成功;后穹隆切开取环成功1例。13例宫内节育器异位均造成子宫损伤,2例伤及肠管,2例伤及大网膜,1例伤及输尿管,1例伤及阑尾。结论:宫内节育器异位易造成盆腹腔脏器损伤,应加强宫内节育器放置技术培训,加强手术护理,放置后应定期检查,一旦发现异位及早取出,同时对损伤的脏器做相应处理。
Objective： To explore the etiological causes of dystopia of IUD, prevention and methods of nursing care. Methods ： The data of 13 cases of IUD dystopia were retrospectively analyzed, Results ： Laparoscope was adopted to take out IUD for 3 patients, successful in 2 patients, failed in one patient. Abdominal operation was performed to take out IUD for 3 patients due to the failure of using hysteroscope. Six patients were performed only abdominal operation to take out IUD successfully. The injuries of uterus were found in all 13 patients with dystopia of IUD. Intestinal injury in 2 patients, injury of greater omentum in 2 patients, injury of ureter in one patient and injury of appendix in one patient. Conclusion： Dystopia of IUD is liable to cause injuries of organs in pelvic and abdominal cavities. Training on placement of IUD should be emphasized and postoperative nursing care must be paid attention to. Regular check - up should be done and IUD has to be taken out as soon as possible once clear diagnosis of dystopia is made.
