老年患者急性胆囊穿孔21例诊治体会

The Experience of Diagnosis and Treatment of 21 Cases with Acute Perforation of Gallbladder in Old Patients

目的：通过对老年急性胆囊穿孔患者21例临床资料的回顾分析了解其临床特点，以期能尽早诊断，正确施治，减少死亡率。方法：对我院2001年1月-2007年12月共收治21例老年急性胆囊穿孔患者临床资料进行回顾性分析。结果；本组21例患者无一死亡，术后切口感染2例，肺部感染1例，急性心衰2例。结论：老年急性胆囊穿孔因其生理及病理特点，临床表现不典型，病情进展快，只有早期诊断，做好围手术期处理，选择适时手术，加强术后管理，才能降低死亡率，减少并发症。

Objective： To analyze the clinical characteristics of 21 cases with acute perforation of gallbladder so as to timely diagnose and reduce mortality. Methods： Respectively analyzed the clinical irdormation of 21 old patients with acute perforation of gallbladder. Results： No death was occurred in these patients. 2 cases had wound infection after operation, 1 case had pulmonary infection and 2 eases had acute heart failure. Conclusions： The clinical manifestation of acute perforation of gallbladder is not typical and the development is fast. The early diagnosis, good treatment during perioperative period, the timely operation and the strengthening of postoperative management can decrease mortalityrate and reduce complications.
