射频等离子手术在治疗鼾症及阻塞性睡眠呼吸暂停低通气综合征中的应用 被引量:2

摘要 目的:研究双极射频等离子手术系统在治疗鼾症和阻塞性睡眠呼吸暂停低通气综合征(OSAHS)中的应用及疗效。方法:280例患者包括单纯鼾症76例,OSAHS204例,分别行双极射频等离子双下甲、软腭及舌根黏膜下消融术,对照分析术前及术后患者症状、多导睡眠图(PSG)、纤维喉镜及CT等结果。结果:280例患者单纯射频等离子治疗软腭62例,单纯治疗双下甲14例,软腭+舌部治疗49例,软腭+双下甲治疗89例,软腭+舌部+双下甲治疗67例。术后随访258例,症状基本消失45例,症状明显减轻114例,症状减轻84例,疗效不明显15例。98例患者术后6个月内行常规专科检查及纤维喉镜检查,示上气道狭窄明显解除,腔径扩大,软腭上提、紧张度增强。18例术前行CT检查的患者于术后1个月行CT复查,舌根后突均有明显回缩。46例OSAHS患者于手术后6个月做PSG监测,呼吸暂停低通气指数由术前(38.13±8.42)min降至(12.98±6.45)min,呼吸暂停总时间由(48.26±12.48)min降至(15.86±6.43)min,最低血氧饱和度由(70.56±12.23)%升至(89.46±14.55)%,鼾声指数由47.79±12.86降至6.34±2.03,均有明显改善。治愈率22%,显效率52%,有效率22%,无效率4%,总有效率96%(44/46)。术后患者不良反应轻,无严重并发症出现。结论:双极射频等离子手术治疗鼾症和轻、中度OSAHS有很好的可行性、有效性和安全性。
