高血压脑出血术后再出血防治对策

Prevention and treatment of recurrent hemorrhage of postoperative hypertensive intracerebral hemorrhage

目的探讨高血压脑出血术后再出血的原因、诊断及防治经验，提高对本病的认识，以便及时发现处理。方法回顾我院1998年1月至2007年1月高血压脑出血术后再出血33例的临床资料。结果高血压脑出血术后再出血发生率11、1％（33／296），二次手术清除血肿者26例，保守治疗7例，预后良好率63．6％（21／33），预后不良率36．4％（12／33）。结论术中止血彻底和术后控制血压平稳对预防再出血意义重大。

Objective To summarize the cause,diagnosis, treatment and prevention of recurrent hemorrhage of postoperative hypertensive intracerebral hemorrhage（HICH）. So that it can be diagnosed in the early phase and treated in time. Methods Thirty -three cases of recurrent hemorrhage of postoperative HICH were analyzed retrospectively from January 1998 to January 2007. Results The rate of recurrent hemorrhage of postoperative HICH was 11.1% （33/296） ,26 cases of recurrent hemorrhage of HICH were operated once again. 7 cases of recurrent hemorrhage of HICH used conservative treatment. The rate of good prognosis was 63.6 % （21/33） ,the rate of bad prognosis was 36.4% （ 12/33 ）. Conclusion Appropriate hemostasis in operation and postoperative blood pressure control are important meanings in preventing recurrent hemorrhage.
