人巨细胞病毒被膜磷蛋白pp65的检测及其应用 被引量:2

摘要 目的:建立一种快速简便诊断肾移植受者人巨细胞病毒(HCMV)活动性感染的方法。方法:运用免疫组织化学的催化信号扩增法检测外周血白细胞中的人巨细胞病毒被膜磷蛋白pp65,巨细胞病毒信使核糖核酸(pp67-mRNA)检测法作比较。结果:检测105例肾移植受者中,HCMVpp65抗原阳性45例,pp67-mRNA检测阳性40例,pp65抗原阳性细胞数为(70±43)个/2×105个WBC,而有症状的CMV病25例,抗原阳性细胞指数为(85±44)个/2×105个WBC。对照组100名健康人,pp65抗原检测全为阴性。pp65的敏感性、特异性分别是100%、88.9%。结论:该法敏感,简便,可作为肾移植术后HCMV病的早期诊断并可指导抗病毒治疗。 Objective: To establish a new method for the active human cytomegalovirus(HCMV) infection in renal transplant recipients.Methods:The expression of human cytomegalovirus phosphoprotein 65(HCMV pp65) antigen in peripheral blood leucocytes was detected by immunohistochemistry method and catalyzed signal amplification(CSA),in comparison to detection of HCMV pp67-mRNA in blood.Results:Of 105 renal transplant recipients,45 had HCMV pp65 positive cells with a HCMV pp65 antigen value of(70±43)/2×105WBC.Among them,40 were positive for pp67.Twenty-five recipients suffered from symptomatic HCMV infection with antigen value of(85±44)/2×105WBC.In 100 healthy people,the results of pp65 and pp67 were all negative.The sensitivity and specificity were 100% and 88.9%,respectively.Conclusion:This method provides a sensitive and convenient approach to the diagnosis of HCMV infection and can guide the antiviral treatment.
