三维CT血管造影在脑膜瘤诊断中的价值——与MRI对照观察 被引量:1

摘要 应用三维CT血管造影(3D-CTA)对4例脑膜瘤(上矢状窦旁、大脑镰、鞍结节、三叉神经半月节各1例)进行了检查,并与MRI对照观察。结果显示:4例脑膜瘤在3D-CTA图象上立体结构鲜明,与周围血管、骨结构立体解剖关系显示清楚;MRI仅显示肿瘤的二维结构,需结合数字减影血管造影(DSA)才能判定肿瘤相关血管的来源和走行。结论:3D-CTA能清楚显示脑膜瘤的立体结构和与周围血管、骨结构的立体解剖关系,通过骨切削技术可模拟颅骨开瓣和手术入路。 Objective: This paper was to study the value of 3DCTA in the diagnosis of meningioma. Methods: We examined 4 cases of meningioma: 1 case located along superior sagittal sinus; 1 case in ceredral falx; 1 case in tubercle of sellae, and another one in trigeminal nerve semilunar ganglin. All were examined by 3DCTA and compared with MRI. Results: 3DCTA could display the three dimensional structure of the 4 meningiomas and demonstrated the stereoanatomic relationship with surrounding blood vessels and bone structure. MRI could display the 2Dstructure of the tumor only, and DSA was necessary for the judgement of the origin and course of its corelatived blood vessels. Conclusion: 3DCTA can display the threedimensional structure of meningioma and demonstrate the stereoanatomic relationship with surrounding blood vessels and bone structures clearly. We can simulate the cranioctomy with bone flap and surgical approach through the technique of serially erasing the bone images. 
