B超测量腹内脂肪厚度在代谢综合征诊断中的应用价值 被引量:14

摘要 目的探讨B超测量腹内脂肪厚度(VFT)在代谢综合征诊断中的应用价值。方法对226例内分泌科住院患者用B超测量VFT,其中45例男性和36例女性同时进行螺旋CT检查。分析B超测量VFT和CT测量腹内脂肪面积(VFA)、腰围(WC)、体质指数(BMI)之间的相关性,并用接收者工作特征(ROC)曲线分析VFT预测代谢综合征的最佳截断点。结果VFT、WC、BMI与VFA均呈正相关(P<0.01);VFT与VFA的相关系数>WC>BMI。男性VFT为55cm对诊断代谢综合征的敏感度为0.79,特异度为0.77,曲线下面积为0.84(P<0.01);女性VFT为42cm对诊断代谢综合征的敏感度为0.81,特异度为0.75,曲线下面积为0.86(P<0.01)。结论B超测量VFT是评估腹内脂肪堆积的可靠指标,可代替WC用于代谢综合征的诊断。 Objective To investigated the feasibility of using ultrasonograpy (US) to assess abdominal obesity and metabolic syndrome (MS), Methods Visceral fat thickness (VFT) was measured by US in 226 patients, of whom 45 males and 36 females were given spiral computed tomography (CT) measurement in the meantime. We determined the correlation between US -measured VFT, visceral fat area (VFA), waist circumference (WC) and body mass index (BMI), and analyzed, by receiver operating characteris curve ( ROC), the best cut - off point of VFT forecast of MS. Results VFT , WC and BMI had positive correlation with VFA, and the correlation coefficients of VFT and VFA were higher than that of WC and BMI ( male r =0 80, female r =0 77, P 〈0.01 ). The sensitivity, specificity and under - curve area of men VFT 42 cm in MS diagnosis were 0 79, 0 77, 0 84, respectively ( P 〈0. 01), those of women VFT 42 cm were 0 81, 0 75, 0 86, respectively ( P 〈 0.01 ). Conclusion US VFT measurement, a reliable indicator in assessing visceral fat accumulation, can be used as a substitute for WC in MS diagnosis.
