带蒂大网膜在外伤性肾破裂修复术中的应用

Application of pedunculated greater omentum in the repair of traumatic renal rapture

目的探讨外伤性肾破裂的保肾方法及大网膜在保肾手术中的应用。方法对28例外伤性肾破裂进行手术修复,其中行修补术24例,行部分切除术4例;同时在手术中应用带蒂大网膜覆盖肾创面。结果28例病人全部治愈出院,经5个月至15年的随访,疗效满意。结论对于外伤性肾破裂,除肾动、静脉损伤已无法修复,或肾脏碎裂严重,需行肾切除术外,均应尽量采用肾修补术或肾部分切除术。在肾修复手术中,应用带蒂大网膜覆盖肾创面对肾伤口的愈合有很大的促进作用。

Objective To explore the reserving methods of traumatic renal rupture and the application of pedunculated greater omentum in the operation of reserving kidney. Methods Repair surgery was performed to 28 cases with traumatic renal rupture. Among them, repair was performed in 24 cases and partial resection of kidney was performed in 4 cases. At the same time, pedunculated greater omentums were used to cover the wound surface of kidney during operation. Results 28 cases were cured to discharge from hospital. Follow-up time was from 5 months to 15 years. The curative effects were satisfactory. Conclusion Except in the renal artery, vein can not be repaired or severe injure of kidney, the renal repair or partial resection should be used as far as possible for patients with traumatic renal rupture. Renal wound surface covered with pedunculated greater omentum has promoted action for renal wound healing during operation of renal repair.
