高血压脑出血合并双侧脑疝患者治疗体会 被引量:1

摘要 目的探讨高血压脑出血合并双侧脑疝患者的治疗方法。方法治疗高血压脑出血合并双侧脑疝患者73例,应用微创碎吸术+开颅血肿清除术治疗19例,单纯微创碎吸术16例,开颅手术25例,保守治疗13例。比较4组患者临床疗效。结果脑疝恢复率:3个手术治疗组均显著优于保守治疗组(P<0.01),微创碎吸术+开颅血肿清除术组优于单纯微创碎吸术组和开颅血肿清除术手术组,差异有统计学意义(P<0.05)。随访6个月,微创碎吸术+开颅血肿清除术组预后优于单纯微创碎吸术组和开颅血肿清除术手术组,差异有统计学意义(P<0.05);3个手术治疗组预后均显著优于保守治疗组(P<0.01)。结论高血压脑出血合并双侧脑疝患者保守治疗病死率极高,手术治疗可显著降低病死率,改善患者预后。应用微创碎吸术+开颅血肿清除术优于单纯微创碎吸术或开颅血肿清除术。
