摘要
目的调查分析健康特勤人员左室高电压的特点,为特勤人员心血管功能专项医学鉴定提供参考依据。方法采用12导联同步心电图仪记录分析2631名特勤人员QRS波电压。结果特勤人员左室高电压阳性率为4.68%(123/2631),其中RV5>2.5mV者占52.85%(65/123),RV5+SV1>4.0mV者占21.14%(26/123),RV5>2.5mV合并RV5+SV1>4.0mV者占23.58%(29/123),RV5>2.5mV合并RⅡ>2.5mV、RⅢ>1.5mV、RavF>2.0mV者占1.63%(2/123),RV5+SV1>4.0mV合并RⅡ>2.5mV、RⅢ>1.5mV、RavF>2.0mV者0.81%(1/123)。除单项QRS高电压外,QRS时间、心电轴以及ST-T正常,均不符合左室肥厚记分法标准,并且临床上亦无引起左室肥厚的病理因素存在。左室高电压多见于年轻人,其中<30岁占60.16%(74/123),另一个引起左室高电压的因素是早期复极综合征,占57.72%(71/123)。结论特勤人员左室高电压阳性率较高,主要表现为V5导联R波增高,年轻人及早期复极综合征是引起左室高电压的重要原因,应注意与真正左室肥厚鉴别。
Objective To investigate the characteristics of left ventricle hypervotage of healthy special service personnel and provide the reference of medical certification of cardiovascular function. Methods QRS of 2631 person of special service was recorded and analyzed with 12-lead synchronous cardiograph, Results The positive rate of special service personnel was 4.68% (123/2 631 ) , in which Rv5〉2.5 mV accounted for 52,85% (65/123) , Rv5+Sv1〉4.0 mV 21.14% (26/123) , Rv5〉2.5 mV combined with Rv5+Sv1〉4.0 mV 23.58% (29/123) , Rv5〉2.5 mV combined with RⅡ〉2.5 mV, RⅢ〉1.5 mV and RavF〉2.0 mV 1.63% (2/123) , Rv5+Sv1〉4.0 mV combined with RⅡ〉2.5 mV, RⅢ〉1.5 mV and RavF〉2.0 mV 0.81%(1/123).Except QRS hypcrvotage, normal QRS time, axis and ST-T were not included in the criteria of diagnosis of left ventricle hypertrophy. There were no pathological factors causing left ventricle hypertrophy clinically, The left ventricle hypervotage occurred commonly in young people, in whom 〈30 years old accounted for 60,16%(74/123), The early repolarization syndrome was another cause for left ventricle hypervotage, accounting for 57.72% (71/123). Conclusion The positive rate of left ventricle hypervotage in the special service personnel is high. High R in V5 is mainly present. The young people and the early repolarization syndrome are main causes. It should be differentiated from the ventricular hypertrophy.
