摘要 目的:通过检测分泌性中耳炎(OME)中耳渗液中白介素5(IL-5)和嗜酸粒细胞阳离子蛋白(ECP)含量,探讨OME发病机制中的变态反应。方法:选取OME患者41例(56耳),以变应原皮肤点刺试验结果将其分变态反应组(17例,26耳)和非变态反应组(24例,30耳)。采用荧光免疫法测量中耳渗液中ECP含量,ELISA法测定IL-5浓度,并进行统计分析。结果:变态反应组患者中耳渗液IL-5和ECP含量均明显高于非变态反应组(P<0.05)。变态反应组中耳渗液ECP与IL-5呈线性相关性(r=0.863,P<0.05)。结论:伴有变应性体质的OME患者中耳渗液中IL-5和ECP水平明显升高,提示以嗜酸粒细胞活化为标志的Ⅰ型变态反应在OME的发病机制中发挥一定作用。 Objective: To investigate the possible relationship between allergy and otitis media with effusion (OME) by the eosinophil cationic protein (ECP) and interleukin 5 (IL-5) level in the middle ear effusion (MEE) of patients with persistent OME. Methods: Fourty-one patients with persistent OME were included and allergy was confirmed by Skin Prick Test (SPT). The concentrations of ECP and IL-5 in the MEE were measured by ELISA and compared allergy positive group with aller- gy negative group. Results : The mean levels of ECP and IL-5 in the MEE were significantly higher in allergy positive group than allergy negative group (P 〈 0.05). ECP level of MEE was positively con'elated with IL-5 in allergy positive group ( r = 0. 863, P 〈 0.05). Conclusion:OME is a sign of allergic inflammation in the middle ear that is associated with a concomitant release of ECP and IL-5 into the effusion in individuals with allergy demonstrated by SPT.
