胃癌患者外周血骨桥蛋白检测的临床意义

Clinical significance of osteopontin in peripheral blood of patients with gastric cancer

目的:探讨外周血骨桥蛋白(osteopontin,OPN)作为胃癌肿瘤标志物检测的临床意义。方法:应用酶联免疫吸附试验(ELISA)和放射免疫分析技术(RIA)检测63例胃癌患者手术前后外周血OPN和癌胚抗原(carcinoembryonic antigen,CEA)的质量浓度,对照组为40例正常健康志愿者。结果:胃癌患者外周血OPN和CEA检测的敏感性分别为69.8%和57.1%,特异性分别为92.5%和87.5%,联合检测的敏感性为77.8%。OPN和CEA的质量浓度与胃癌的临床分期、淋巴结和器官转移有关,与胃癌的组织学分型及大小无关。治疗前后两种标志物的质量浓度具有显著差异。结论:OPN是一种较为理想的胃癌肿瘤标志物,与CEA联合检测能提高胃癌诊断的敏感性。

Objective To evaluate the clinical significance of osteopontin （OPN） as a tumor marker in peripheral blood of patients with gastric cancer. Methods Serum levels of OPN and carcinoembryonic antigen （CEA） in gastric cancer （n = 63） in pre- and post-operation and in control group （n =40） were detected by ELISA and RIA methods. Results The sensitivity of OPN and CEA in patients with gastric cancer was 69. 8% and 57. 1%, and the specificity of OPN and CEA was 92.5% and 87.5%, respectively. The sensitivity of OPN combined with CEA was 77.8%. Serum levels of OPN and CEA were related with PTNM stage, lymph node involvement and presence of distant metastases and not related with histological type and size. Pre-and post-operative levels of the two markers were obviously different. Conclusion OPN, a potential tumor marker, improves the sensitivity of diagnosis of gastric cancer when combined with CEA.
