脑白质病变对阿尔茨海默病锥体外系体征的影响 被引量:1

摘要 目的探讨脑白质病变(WMLs)对阿尔茨海默病(AD)患者锥体外系体征(EPSs)的影响。方法临床收集轻度AD患者77例,根据MRIT2加权像是否合并WMLs分为单纯AD组34例和AD合并WMLs组43例,采用简明精神状态量表(MMSE)、统一帕金森病评定量表(UPDRS)运动部分和日常生活活动量表(ADL)分别进行认知功能、EPSs和日常生活能力的评估。结果与单纯AD组相比,AD合并WMLs组ADL评分、UPDRS运动部分总评分以及强直、运动迟缓、轴性损害单项评分均显著升高(P<0.05);控制了年龄、血管危险因素和认知功能的影响后,运动迟缓、轴性损害与AD合并WMLs仍具有相关性。结论WMLs可促进AD患者EPSs的恶化,积极预防WMLs有助于减轻AD患者的EPSs。 Objective To explore the influences of white matter lesions (WMLs) on extrapyramidal signs of Alzheimet disease(AD). Methods Seventy-seven mild AD patients were classified into pure AD group and AD and WMLs group according to MRI results. All patients were assessed with mini-mental state examination ( MMSE ) , motor portion of unified Parkinson' s disease rating scale(UPDRS) and activities of daily living(ADL). Results Compared with pure AD group, the patients in AD and WMLs group had higher scores in ADL, motor portion of UPDRS and higher scores of rigidity, bradykinesia and axial function. Ajusting for age, vascular risk factors and cognitive function, bradykinesia and axial impairment were associated with AD patients with WMLs. Conclusions WMLs may deteriorate EPSs of AD patients. Preventing WMLs may be helpful to improve EPSs of AD patients.
