纤维支气管镜在医院获得性肺炎治疗中的作用

Therapeutic Effect of Bronchofibroscope in Treatment of Hospital Acquired Pneumonia

目的:探讨纤维支气管镜在医院获得性肺炎治疗中的作用。方法:将收治入院的医院获得性肺炎患者205例随机分为两组,使用纤维支气管镜治疗称纤支镜组103例,不使用纤维支气管镜治疗的为对照组102例。结果:纤支镜组细菌培养污染率为0,平均使用抗生素时间13 d,平均住院时间为19 d,住院费用平均为5 547元(不包括气管切开前基础疾病的治疗费用)。对照组细菌培养污染率为37.2%,平均使用抗生素时间21 d,平均住院时间为29 d,住院费用平均为10 644元(不包括气管切开前基础疾病的治疗费用)。结论:经纤维支气管镜,清除支气管分泌物并向感染支气管肺段内注入有效抗生素,是治疗气管切开术后肺部感染的高效、经济、适用的方法。

Objective： To evluate the therapeutic effect of bronchofibroscope in treatment of hospital acquired pneumonia. Methods ：205 patients were divided into 2 groups randomly, one group were treated with bronchofibroscope （treatment group） ,the another group were not treated with bronchofibroscope （control group）. Then the contrasting analysis was carried out from pathogenic bacteria of pulmonary infection, therapeutic effect, length of stay and cost of hospitalization. Results ：In treatment group,the results showed that inquination rate of segmental bronchus was zero by cultured bacteria of excretion and examed their susceptibility to antibiotics. The average time of administrating antibiotics were 13 days,average stay were 19 days ,average cost of hospitalization was 5547 yuan. In control group, inquination rate was 37.25% ,antibiotic were used for 21 days ,stay in hospital for 29 days and cost of hospitalization was 10644 yuan averagely. Conclusion ：It is a effective and economic method to treat pulmonary infection after tracheotomy to clear the excretion of segmental bronchus by bronchofibroscope ,and to irrigate infected bronchopulmonary segment with sensitive antibiotics by the results of bacterial culture and examination of susceptibility to antibiotics.
