尿动力学在尿频患者病因诊断中的临床应用价值(附267例报告)

The clinical value of urodynamics in the diagnosis of frequency in patients(267 cases reported)

目的:评价尿动力学在尿频患者病因诊断中的重要价值。方法:对267例不同年龄的尿频患者行尿流率、压力容积、压力-流率、前列腺压和肌电图检查,根据检查结果,行相应的临床治疗措施,比较治疗前后排尿次数的变化。结果:55.9%的患者有膀胱出口梗阻,行手术或尿道扩张有效率为98.7%;21.8%的患者存在不稳定膀胱,行药物治疗有效率为86.4%;19.6%的患者有逼尿肌反射亢进,行药物及膀胱训练有效率为65.6%;2.7%的患者有逼尿肌无反射,行膀胱造瘘及膀胱训练有效率为100%。结论:尿动力学检查揭示尿频的深层次病因,在尿频患者的诊断、鉴别诊断及治疗方式的选择上具有极其重要的意义,值得临床推广应用。

Objective： To evaluate the significant value of urodynamics in the diagnosis of the frequency in patients. Methods： To exam the flow rate, pressure volume studies, pressure flow studies, prostatic pressure, electromyogram of 267 cases of different ages frequency. Then they were given alternative strategies to treat according to inspection results , and compared with the before. Results： 55.9% of patients were bladder outlet obstruction, the effectiveness of surgery or urethra expansion was 98.7% ;21.8% of patients were unstable bladder, effectiveness of drug treatment was 86.4% ; 19.6% of patients were overactive bladder, effectiveness of drug treatment and bladder training was 65.6% ; 2.7% of patients were detrusor areflexia, effectiveness of punction vesicae and bladder training was 100%. Conclusion： Urodynamics examination revealed the deepseated causes of frequency, it was very great significance to made the diagnosis, differential diagnosis and the choice of treatment, worthy of clinical application.
