Expression of survivin protein in esophageal squamous cell carcinoma and its significance

目的:探讨Survivin在食管鳞癌中的表达及其临床意义。方法:采用免疫组织化学检测47例食管鳞癌组织和10例正常食管黏膜的Survivin蛋白的表达,探讨其与食管鳞癌临床病理特征及预后之间的关系。结果:Survivin阳性表达定位于肿瘤细胞胞浆内。47例食管鳞癌组织Survivin阳性表达率为93.6%。在正常食管黏膜无阳性表达。Survivin表达水平与组织学分化程度有相关关系,高分化鳞癌中低表达者居多,而中分化鳞癌中高表达者居多,二者呈负相关(P<0.05)。总体生存率单因素分析显示Survivin高表达组生存预后明显差于低表达组,有显著性意义(P<0.05)。Logistic多因素回归分析显示Survivin表达水平为独立预后因素(P<0.05)。Survivin表达水平与食管癌治疗失败(手术后3年内复发和转移死亡)有明显相关关系,高表达组发生治疗失败的病例明显增多(P<0.01)。结论:Survivin表达水平与组织学分化程度有相关关系,为食管癌的独立预后因素;Survivin高表达预示食管癌治疗失败的机会增大。

Objective. To investigate the expression of survivin proteins in esophageal squamous cell carcinoma and its clinical significance. Methods. The expression of survivin protein was detected by immunohistocbemical staining method in 47 esophageal squamous cell carcinoma samples and 10 normal epithelial tissues of esophagus. The expression of survivin proteins and its correlation with clinical pathological characteristics and prognosis were assessed. Results： The expression of survivin protein was located in the cytoplasm of cancer cells. The positive expression rate of survivin protein in 47 cancerous tissues was 93.6%. No expression of survivin protein was detected in 10 non - cancerous normal tissues of esophagus. The expression of survivin was correlated with histological differentiation degree （ P 〈 0.05 ）. Lower expression of survivin in well - differentiated carcinomas was much more than that in middle -differentiated carcinomas. There was an inverse correlation between the survivin expression and histological dif- ferentiation degree. The univariate analysis showed that the survival rates of patients with higher level of expression of survivin were significantly lower than that of patients with lower level of expression of survivin （ P 〈 0.05 ）. The Logistic multivariate analysis showed that the expression level of survivin protein was independent prognostic factor. There was a significant correlation between the expression of survivin protein and treatment failure （ P 〈 0.01 ）. Conclusion： The expression of survivin was correlated with histological differentiation degree. The expression level of survivin protein is independent parameter for esophageal squamous cell carcinoma, and higher expression of survivin may predict the higher incidence of treatment failures.
