摘要
目的:探讨基质金属蛋白酶(Metalloproteinase,MMP)-14、-2、基质金属蛋白酶组织抑制因子(Tissue inhibitor ofmetalloproteinase,TIMP)-2在胃癌、癌旁组织及胃溃疡中的表达情况及相互关系。方法:用免疫组织化学方法(SP法)检测30例胃癌手术病人及20例胃镜活检胃溃疡病人中MMP-14、MMP-2、TIMP-2的表达。结果:(1)胃癌组织中,TIMP-2、MMP-2及MMP-14表达阳性率分别为43.3%、76.7%、53.3%,胃癌癌旁组织中分别为56.7%、46.7%和40%,胃溃疡组织中分别为70%、45%和30%。(2)在胃癌中,TIMP-2的阳性表达与癌旁组织及胃溃疡组织比较有统计学差异(P<0.01),MMP-2阳性表达与溃疡组织之间比较有统计学差异(P<0.05)。(3)在癌组织中,MMP-2、MMP-14和TIMP-2表达均与浆膜是否受侵、淋巴结有无转移和TNM临床分期密切相关(P<0.05)。(4)双变量相关分析,在胃癌组织中,MMP-14和MMP-2阳性表达呈正相关(rs=0.447,P>0.05),而TIMP-2阳性表达与前两者均呈显著负相关(rs=-1.0,P<0.01)。结论:(1)MMP-1在胃癌中高表达。(2)MMP-14和MMP-2阳性表达呈正相关,是胃癌的侵袭转移性因素之一。(3)MMP-14表达上调的同时激活了更多的MMP-2,增加了胃癌向周围组织侵袭转移的能力。
Objective:To detect the expression of MMP-14,MMP-2 and TIMP-2 in gastric ulcer tissues, gastric cancer and tissues around gastric cancer. Methods:The expression of MMP-14, MMP-2,and TIMP-2 were detected by S-P immunohistochemistry in gastric ulcer tissues,gastric cancer tissues and around tissues from operated patients of gastric cancer (n=30) and gastric ulcer patients(n=20)who were diagnosed by stomach lens. Results:(1) The expression rates of TIMP-2, MMP-2 and MMP-14was 43.3%,76.7% and 53.3% respectively in gastric cancer tissues and 56.7%,46.7% and 40%in tissues around it,but 70%,45% and 30% in gastric ulcer tissues. (2) The expression of TIMP-2 in gastric cancer was lower than that in tissues or that in gastric ulcer tissues(P〈0.01 ).The expression of MMP-2 in gastric cancer was higher then that of gastric ulcer (P〈0.05).(3) The expression of MMP-2,MMP-14 and TIMP-2 in gastric cancer tissues were closely correlated to the serosa invaded, lymph node metastasis and TNM stages(P〈0.05). (4) There was positive correlation between the expression of MMP-2 and MMP-14 in gastric cancer(rs=0.447,P〉0.05).The expression of TIMP-2 was negatively correlated with the expression of MMP-2 or MMP-14(rs=-1.0,P〈0.01)in gastric cancer. Conclusions:(1)The MMP-14 in gastric cancer is over expressed.(2)MMP -14 was one of invading and metastasis factors in gastric cancer and could judge prognosis of it. (3)The high expression of MMP-14 and MMP-2 showed that increased expression of MMP-14 resulted in more MMP-2 activation and increased ability of invading and metastasis in the tissues around gastric cancer. (4)TIMP-2 plays an important role in resisting MMP-14 in gastric cancer and gastric ulcer tissues.
