三种电流强度下神经刺激仪定位肌间沟臂丛阻滞临床观察

摘要 目的探讨在三种不同电流强度下神经刺激仪辅助定位肌间沟法臂丛神经阻滞的临床效果。方法选择拟择期上肢手术患者60例,随机分为3组,每组20例,A组在神经刺激仪电流强度为0.2mA仍有肌肉震颤时注射局麻药,B组在0.3mA仍有肌颤时注射局麻药,C组在0.4mA有肌颤时即注射局麻药,局麻药均为0.375%罗哌卡因30ml,观察操作时间(均为同一人操作)、阻滞起效时间、神经阻滞的成功率及不良反应。结果A组在操作时间上长于B组和C组,阻滞起效时间C组长于A组和B组,神经阻滞成功率及不良反应3组无明显差异。结论采用神经刺激仪定位肌间沟法臂丛神经阻滞时,在电流强度为0.3mA仍有肌颤时注射局麻药最合适。 Objective To observe the clinical effect of intersclance brachial plexus blockage by nervous stimulator created muscle vibrating with three current intensity. Methods 60 cases undergoing upper limb surgery were randomly divided into 3 groups,20 cases each group. Group A and B and C separately receiving 0.375% ropivacaine 30 ml in interscalence brachial plexus block by nervous stimulator created muscle vibrating with current intensity of 0.2mA,0.3mA, 0.4mA.The time of block operation and effect starting, success rate and untoward effect were recorded. Results The time of block operation was significantly longer in group A,and the time of block effect starting was significantly longer in group C. Conclusion 0.3mA of current intensity is more suitable than others when nervous stimulator creating muscle vibrat in interscalence brachial plexus blockage with 30ml of 0.375% ropivacaine.
