双腔起搏不同房室传导间期对老年慢性心力衰竭患者心功能的影响

Effect of dual-chamber pacemaker on cardiac function in elderly chronic heart failure

目的观察双腔起搏时两种不同的房室传导间期对老年慢性心力衰谒患者心功能的影响。方法26例植入双腔起搏器的老年慢性心力衰竭患者,采用自身对照分为长AV间期组(DDD-l)和短AV间期组(DDD-s),分别观察1周,行超声心动图检查,比较两组左室射血分数(LVEF)、左室舒张末直径(LVEDD)、左室收缩末直径(LVESD)的变化。结果DDD-s组LVEF(50.92±6.99)较DDD-l组LVEF(53.46±4.87)明显降低,而LVEDD、LVESD则两组间无改变。结论由于DDD-s的完全右室起搏,可引起老年慢性心力衰竭患者心功能恶化。

Objective To study the effect of dual-chamber pacemaker on cardiac function in elderly chronic heart failure. Methods 26 patients with dual-chamber pacemaker were divided into two groups by self control,one is long AV delay group DDD-1 group,the other is short AV delay group DDD-s group. After one week observation of each group,we compared results of LVEF,LVEDD and LVESD. Results With paired t-test,the LVEF of DDD-s group is obviously decrease,50.92±6.99 vs 53.46±4.87. Conclusions Because DDD-s group is completely right ventricular pacing,it may decrease heart function in elder chronic heart failure patients.
