椎弓根螺钉内固定治疗不稳定性Hangman骨折

摘要 目的评价椎弓根螺钉内固定治疗不稳定性Hangman骨折的临床应用价值。方法回顾分析我科自2002年以来收治的9例不稳定性Hangman骨折患者,所有患者均采用C2~3椎弓根螺钉侧块钉板系统内固定治疗。结果经过12~38个月随访,X线片显示骨折愈合,复位良好。无动脉损伤及其他手术并发症。1例术前Frankel分级为D级的患者术后恢复至E级。结论椎弓根螺钉内固定治疗不稳定性Hangman骨折可获得骨折即时复位,固定可靠,是一种理想的治疗方法。 Objective To evaluate the outcomes of pedicle screw fixation in treatment of unstable Hangman's fracture. Methods 9 cases with unstable Hangman's frature were retrospectively analyzed. 5 of them were type II,2 of them were type IIa,and 2 of them were type III. All patients were treated with pedicle screw fixation. Results After 12 to 38 months following-up,X-ray showed all the fractures healed up with satisfactory reduction.No vertebra artery injury or other complications occurred. 1 case of grade D according to Frankle classification recovered to grade E after operation. Conclusion Pedicle screw fixation is an ideal way of treatment of unstable Hangman's fracture,which can provide immediate reduction and satisfactory biomechanical stability.
