摘要
目的通过观察针刀松解法对第3腰椎横突综合征大鼠耐痛阈及脊髓和下丘脑单胺类神经递质的影响,探讨其治疗第3腰椎横突综合征的镇痛机制。方法将SD大鼠随机分为4组,即正常组、模型组、电针组和针刀组,后3组首先建立第3腰椎横突综合征动物模型,后2组再分别于造模后给予电针和针刀松解干预治疗,期间以热辐射法观察大鼠痛阈变化,造模28 d后分别取大鼠下丘脑和脊髓腰段组织以反相高效液相色谱法检测其单胺类神经递质及其代谢产物的含量。结果造模大鼠痛阈较正常组明显降低(P<0.05);治疗后电针组和针刀组大鼠痛阈有显著提高,与正常组比差异不显著(P>0.05)。模型组5-羟吲哚乙酸(5-HIAA)含量显著降低(P<0.01),电针组、针刀组脊髓5-HIAA的含量比模型组明显升高(P<0.01),与正常组差异不显著(P>0.05);针刀组3,4-二羟基苯乙酸(DOPAC)在脊髓中的含量明显低于其余3组(P<0.05);而电针组DOPAC在下丘脑的含量明显高于其余3组(P<0.05);在脊髓和下丘脑5-羟色胺(5-HT)、去甲肾上腺素(NE)和多巴胺(DA)的含量4组间均无明显差异。结论针刀松解法对第3腰椎横突综合征模型大鼠有较好的镇痛作用,其机制可能是通过调节部分中枢单胺类神经递质的代谢、释放而实现的。针刀松解法的综合疗效略好于电针治疗。
Objective To observe the effect of acupotomy on pain threshold and monoamine neurotransmitters in spinal cord and the hypothalamus of rats with the transverse process syndrome of third lumbar vertebrae.Methods Twenty-eight rats were randomly divided into four groups: the normal control group,the model group,the electroacupuncture(EA) group and the acupotomy group.The last three groups were modeled by an operation.The EA group and the acupotomy group were treated respectively.The changes of rats' pain threshold were observed with thermal radiation-resistant method.28 days after modeled,monoamine neurotransmitter contents of hypothalamus and spinal cord were detected with RP-HPLC.Results The value of pain threshold was significantly lower in the model group than in the normal group(P〈0.05) and was higher in the EA group and the acupotomy group than in the model group with no significant differences compared with the normal group(P〉0.05).The contents of 5-HIAA of the spinal cord in the EA group the acupotomy group were significantly higher than that in the model group(P〈0.05),but there was no significant difference compared with the normal group(P〉0.05).The content of DOPAC in the hypothalamus in the EA group were more than that in the other three groups(P〈0.05).The content of DOPAC in the spinal cord in the acupotomy group was significantly lower than that in the other three groups(P〈0.05).The content of 5-HT,NE and DA in both the spinal cord and the hypothalamus were no significant differences among four groups(P〉0.05).Conclusion Acupotomy's analgesic effects on the rat model may through the promotion of metabolism and liberation of some monoamine neurotransmitter.Its' effect may be better than electroacupuncture in this respect.
