正颌外科技术在颌面部骨折治疗中的应用

The Application of Orthognathic Surgery Technology in the Treatment of Maxillofacial Fracture

目的:评价正颌外科技术在颌面部骨折治疗中的方法和效果。方法:对在我院就诊的38例陈旧性颌面骨折,运用正颌外科技术进行治疗,按正颌外科常规,术前摄头颅定位正侧位片,取模;头影测量及模型外科等,术前制作咬颌板,手术均采用标准的正颌外科截骨线进行,精确复位后行坚强内固定,术后颌间固定2周～3周;术后1个月、3个月、6个月复查拍片,观察骨折愈合情况、咬颌关系及开口度等功能恢复情况。结果:38例患者创口均1期愈合,术后经颌间牵引,咬颌关系和外观基本恢复伤前状况。结论:治疗颌面部陈旧性骨折应用正颌外科技术,此法具有稳固性好,咬颌关系稳定、感染率低的优点,目前是治疗颌骨骨折错位愈合较理想的方法。

Objective： To evaluate the effects of orthognathic surgery to treat cases with the dentomaxilloficial deformities following maxillofacial fractures. Methods： According to the regular rules of orthognathic surgery, the 38 patients should he taken cephalometric analysis, impression and model surgery before operation. Then we made occlusion guide plate and individuation dental arch splint. The patients were taken osteotomy by orthognathic surgery. Rigid internal fixation was made after reducing occlusion relation and facial form. Intermaxillary fixation was used from 2 to 3 weeks postoperatively. Results： All of the wound were primary healed. The dentomaxillofacial deformities and disfunction were restored. Conclusion： By the means of orthognathic surgery technology together withrigid internal fixation, the treatment of obsolete fracture of jaws bone can not only recover facial form hut also improve bite relation to the extreme.
