2型糖尿病患者血清炎症因子与胰岛素抵抗的关系 被引量:4

The correlation of inflammatory cytokines and insulin resistance in type 2 diabetes mellitus

摘要 目的观察2型糖尿病(T2DM)患者高敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)及肿瘤坏死因子α(TNF-α)与胰岛素抵抗(IR)的相关关系。方法选择正常对照组80例,T2DM患者162例;将T2DM分为A、B组:A组为单纯T2DM患者组81例、B组为T2DM大血管病变组81例;测定各组血清hs—CRP、IL-6、TNF-α水平。并计算胰岛素抵抗指数(HOMA—IR)。结果T2DM的hs—CRP、IL-6、TNF-α、IR水平明显高于正常对照组。T2DM大血管病变组的hs—CRP、IL-6、TNF-α、IR水平均明显高于单纯T2DM患者组。T2DM组患者hs—CRP、IL-6、TNF-α与IR均呈正相关。结论炎性反应与胰岛素抵抗同步存在,而且与糖尿病及并发症发生密切相关,炎症因子能较好的反映糖尿病患者的病情和胰岛素抵抗程序。 Objective To investigate the relationship between serum levels of high-sensitivity C-reactive protein,inter leukin-6, tumor necrosis factor-α and insulin resistance of type 2 diabetes mellitus ( T2DM ). Methods 80 health people and 162 T2DM patients were chosen. Selecting 81 T2DM patients without macrovascular complications as A ,81 T2DM with macrovascular complications as B. Serum hs-CRP, IL-6 and TNF-α levels in the 3 groups were detected. HOMA-IR were calculated. Results ' The serum levels of hs-CRP, IL-6, and TNF-α in T2DM were significantly increased than that in normal controls. The serum levels of hs-CRP, IL-6, and TNF-α of participants with macrovascular complications were significantly increased than that of participants without macrovascular complications in T2DM. The levels of serum hs-CRP, IL-6 and TNF-α were positively correlated with HOMA-IR in T2DM. Conclusion Inflammatory response and insulin resistance are correlated positively with the initiation and development of T2DM. Inflammatory cytokines may reflect the state of T2DM and insulin resistance preferably.
