白介素-8在分泌性中耳炎中的临床意义

Clinical significance of IL-8 in secretory otitis media

目的对分泌性中耳炎患者的中耳积液标本进行白介素-8（interleukin-8，IL-8）含量检测分析，以探讨其在分泌性中耳炎中的临床意义。方法用夹心酶联免疫吸附法检测患者中耳积液、血清中的IL-8含量以及对照组血清中的IL-8含量。结果IL-8在中耳积液中阳性表达率为89、16％，中耳积液中IL-8的含量明显高于血清中的含量（P〈0．01），急性期组的含量较慢性期组高（P〈0．05），浆液性积液中的含量较粘液性积液高（P〈0．05）；实验组血清中IL-8含量较对照组血清高（P〈0．05）。结论分泌性中耳炎中耳积液中的IL-8可能是由中耳腔局部产生的，而非单纯由血液中渗透而来，IL-8在疾病早期参与机体的防御反应，促进浆液性中耳积液产生，深入了解这一作用对寻找分泌性中耳炎有效的治疗方法可能有重要意义。

Objective This experiment measured and analyzed the concentration of IL-8 in middle ear effusions （MEE）of patients with secretory otitis media（SOM） to explore its clinical significance in secretory otitis media. Methods A commercially available enzyme-linked immunosorbent assay（ELISA） incorporating a biotinylated monoclonal antibody probe was utilized for the quantitative measurement of IL-8 in the MEE and serum samples of the patients and normal persons. Results Positive results of IL-8 were found in 89. 16% of MEE. The concentration of IL-8 in the MEE was remarkably higher than those in the serum samples （P 〈 0. 01 ）. The concentration of IL-8 in the acute period in patients with SOM was higher than that in the chronic period （P 〈 0. 05）. IL-8 concentration of MEE in serous fluid group was higher than that in the mucous fluid group （ P 〈 0. 05）. The concentration of IL-8 in the serum samples of the test group was higher than that of the normal control group （P 〈 0. 05 ）. Conclusion The IL-8 in the MEE seems likely to be produced locally within the tympanum, but not transferred simply from blood. IL-8 might participate in the defensive reaction of organism during the early stages of SOM, and play a role in serous effusion secretion,which may be important in finding a potential treatment method.
