Expressions of KISS-1 and MMP-9 in non-small cell lung cancer and their correlation with metastasis and prognosis

摘要 目的:检测KISS-1和基质金属蛋白酶-9(MMP-9)蛋白在非小细胞肺癌(non-small cell lung cancer,NSCLC)中的表达,旨在探讨其表达水平与转移和预后的关系。方法:采用免疫组化(Elivision)法检测85例原发NSCLC患者和56例对应转移的淋巴结中KISS-1和MMP-9蛋白的表达水平,用图像分析方法测定其积分光密度(integral optical density)。结果:(1)KISS-1在Ⅰ~Ⅱ期NSCLC、无转移NSCLC、原发灶中的表达分别高于Ⅲ~Ⅳ期NSCLC、有转移NSCLC、淋巴结转移灶中的表达(P<0.05);(2)MMP-9在鳞癌、肿瘤直径≤3cmNSCLC、Ⅰ~Ⅱ期NSCLC、无转移NSCLC、原发灶中的表达分别低于腺癌、肿瘤直径>3cmNSCLC、Ⅲ~Ⅳ期NSCLC、有转移NSCLC、淋巴结转移灶中的表达(P<0.05);(3)KISS-1与MMP-9的表达水平呈负相关(r=-0.523,P<0.01);(4)KISS-1蛋白高表达患者的术后5年生存率(20.9%)显著高于低表达患者(2.4%)(P<0.01),而MMP-9高表达患者的术后5年生存率(4.7%)显著低于低表达患者(19.0%)(P<0.05)。结论:KISS-1的弱表达和MMP-9的强表达可能与NSCLC的转移有关,两者的失衡可能是造成转移的一个原因。 Objective To detect the expressions of KISS-1 and MMP-9 proteins in non-small cell lung cancer (NSCLC) and explore their correlation with metastasis and prognosis. Methods The expressions of KISS-1 and MMP-9 proteins were detected in 85 cases of NSCLC and 56 involved lymph nodes by immunohistochemistry and integral optical density was determined by the image analysis system. Results Levels of KISS-1 expression were significantly higher in stage Ⅰ - Ⅱ disease, non-metastatic NSCLC, and primary tumor than in stage Ⅲ- Ⅳ disease, metastatic NSCLC, and involved lymph node, respectively (P 〈 0.05 for all comparisons), while those of MMP-9 expressions were significantly higher in adenocarcinoma, a tumor size 〉 3 cm in diameter, stage Ⅲ-Ⅳ disease, metastatic NSCLC, and involved lymph node than in squmous cell carcinoma, a tumor size ≤3 cm in diameter, stage Ⅲ-Ⅳ disease, metastatic NSCLC, and primary tumor (P 〈 0.05 for all comparisons). The level of KISS-1 expression was negatively related with that of MMP-9 (r = -0.523, P 〈 0.01). The 5-year survival rate was markedly greater in the patients with higher KISS-1 expression than in those with low expression (20.9% vs 2.4%, P 〈 0.01), whereas it was evidently lower in the patients with higher MMP-9 expression than in those with low expression (4.7% vs 19.0%, P 〈 0.05). Conclusion Low KISS-1 expression and high MMP-9 expression may be associated with NSCLC metastases.
