含庆大霉素羟基磷灰石涂层假体的体外药物释放试验的药物浓度监测 被引量:1

摘要 目的:探讨含庆大霉素羟基磷灰石涂层假体的体外药物释放试验的药物浓度监测。方法:用仿生溶液法制备出含庆大霉素羟基磷灰石涂层假体,在生理盐水体外释放实验中检测其释放的庆大霉素的浓度和持续时间。采用荧光偏振免疫分析法测定假体庆大霉素体外释放的药物浓度。结果:在庆大霉素体外释放药物浓度监测中,按释放的不同时间点进行测定,共分别测得药物浓度值为:1 h为3.58μg.mL-1,24 h为5.36μg.mL-1,48 h为5.45μg.mL-1,72 h为5.14μg.mL-1,120 h为5.13μg.mL-1,192 h为4.64μg.mL-1,216 h为0.44μg.mL-1,240 h为0.31μg.mL-1。结果显示,体外庆大霉素释放速度快,1 h内就已经达到了金葡菌的最低抑菌浓度(MIC为0.25μg.mL-1),且持续释放到第10天时,药物浓度仍高于MIC。体外庆大霉素释放药物浓度可持续达药物浓度时间约8 d。结论:开展抗生素药物浓度监测分析,可为临床提高合理用药水平及降低关节置换术后的感染提供可靠的依据,确保临床用药安全、合理、有效。 OBJECTIVE: To discuss the monitoring of drug concentration in drug release test of gentamicin - hydroxyapatite coating prosthesis in vitro. METHODS: Gentamicin- hydroapatite coating prosthesis were prepared by using biomimetic solution method. Monitoring the concentrations and the release duration of gentamicin released from the prosthesis by rinsing it in physiologic saline solution. The concentration of gentamicin released from gentamicin - hydroxyapatite coating prosthesis was determined by Fluorescence Polarization Immunoassay. RESULTS: Monitoring the concentrations by time, and the results were as follows: 3.58ug· mL^-1 for 1 h, 5.36ug· mL^-1 for24 h, 5.45ug· mL^-1 for 48 h, 5.14ug· mL^-1 for72 h, 5.13 ug·mL^-1 for 120 h, 4.64 ug·mL^-1 for 192 h, 0.44ug·mL^-1 for 216 h, 0.31ug·mL^-1 for 240 h. The results showed that the gentamicin released rapidly from the Gentamicin - hydroapatite coating prosthesis and it could reach the minmum inhibition concentration (MIC is 0.25 ug· mL^-1 ) of the Staphylococcus within 1 hour, and remained still higher than MIC at the 10th day of sustained release. The theropeutic drug concentrations of gentamicin could lasted for about 8 days. CONCLUSION: Monitoring of concentration of antibiotics could provide credible evidence for enhancing levels of rational use of drugs and reducing the incidence of infection after joint replacement, and ensure safe, rational and effective clinical medication.
