儿科门诊处方差错分析

Analysis of Prescriptions Errors in Pediatric Outpatient Department

目的:汇总、分析门诊药房处方差错类型及原因,提出相应措施杜绝差错的发生。方法:根据笔者多年儿科药房工作经验,系统回顾、归纳分析近几年门诊药房发生的差错类型及原因,并提出相应的预防措施。结果及结论:引起药物差错的原因很多,大致可以分为4类:开方差错、调配差错、患者差错及其他差错。通过建立用药系统监测及差错报告机制,大多数差错是可以避免及预防的。

OBJECTIVE： To collect and analyze the types of and reasons for prescription errors and to pur forward the corresponding countermeasures. METHODS： The types of and reasons for medication errors were reviewed and analyzed systematically based on authors＇ years＇ work in pediatric pharmacy, and the corresponding preventive measures were put forward. RESULTS ＆ CONCLUSION： Medication errors were mainly resulted from prescribing errors, dispensing errors, patients errors and other errors. By establishing medication monitoring and error reporting system, the majority of medication errors can be avoided and prevented.
