110例经皮肾镜碎石术患者的临床麻醉探讨

Anesthesia Management of 110 Patients with Invasive Percutaneous Nephrolithotomy

目的探讨两点法连硬外麻醉在经皮肾镜碎石术中应用的有效性。方法选择110例经皮肾镜碎石术的肾输尿管结石患者,麻醉采用两点法连硬外阻滞麻醉。结果麻醉优良率为96.4%;与麻醉前比,麻醉后30min及术毕后MAP下降明显(P<0.01),麻醉后30min及术毕后MAP比较无显著性差异。与麻醉前比,麻醉后30min HR加快,但无统计学差异;术毕后,与麻醉前及麻醉后30min比,HR明显加快(P<0.01)。患者SpO2变化无统计学差异。结论两点法连硬外阻滞麻醉在经皮肾镜碎石术中起效快、效果确切,但在术中要严密观察呼吸及血流动力学的变化。

Objective To discuss the effect of application waist hard union anaesthesia in PCNL. Methods 110 patients applications after PCNL are choosed in the treatment exercises. Results The anaesthetizes finely rate is 96.4% ;Compared to the before anaesthesia,after the anaesthesia with 30min and the technique finish, MAP drops obviously（P〈 0.01 ）,after the anaesthesia,after 30min and the technique finish, the MAP comparison difference does not have the significance. Compares before the anaesthesia,after the anaesthesia,after 30min,HR speeds up obviously（P〈 0.01 ）, the changes of SpO2 don＇t have the statistics difference. Conclusion The waist hard union anaesthesia in PCNL have the better merits of effect quickly and accurately,but must to be observed the breath and the blood stream dynamics change strictly in the technique.
