CPT-11联合5-FU/CF治疗晚期结直肠癌的研究

Clinical Study of CPT-11 Combined with 5-FU/CF Regimen in Treatment of Patients with Advanced Colorectal Cancer

目的评价CPT-11联合5-FU/CF方案治疗晚期结直肠癌的临床疗效及毒副反应。方法全组29例,CPT-11180mg/m2,静滴90min,d1;CF200mg/m2静滴2h,d1-2,5-FU400mg/m2静推后,5-FU600mg/m2用微量化疗泵持续静推22h,d1-2,每2周重复。治疗2周期后评价疗效。结果29例患者中CR0例,PR11例,SD13例,PD5例,总有效率34.48%。主要毒副反应为迟发性腹泻(Ⅲ/Ⅳ度发生率为34.48%)及中性粒细胞减少(Ⅲ/Ⅳ度发生率为27.59%。结论CPT-11联合5-FU/CF方案治疗晚期结直肠癌有效率高,毒副反应可以耐受,可作为晚期结直肠癌的一线或二线化疗方案。

Objective To evaluate the clinical efficacy and adverse effects of irinotecan（CPT-11 ） combined with 5-FU/CF in treatment of advanced colorectal cancer. Methods 29 patients with advanced colorectal cancer received therapy,CPT-11 180mg/m^2 ivgtt dl, CF 200mg/m^2 ivgtt followed by 5-FU 400mg/m^2 iv and 5-FU 600mg/m^2 ivgtt 22h,d1-2,a cycle was 14 days. Results 29 cases could evaluate efficacy,the complete response and partial response were observed in 0 and 11 cases respectively,The overall response rate was 34.48%. Dose limiting toxicity（DLT）was delayed diarrhea and neutropenial. Conclusion CPT-11 combined with 5-FU/CF is an effective and well tolerated regimen in the treatment of advanced or metastatic colorectal cancer, which can relieve symptoms and improve quality of life of the patients. It can be used as the first-line or second line therapy for advanced or metastatic colorectal cancer.
