妊娠体质量与娠妊结局的关系

The relationship between maternal Body Mass Index and pregnancy outcome

目的探讨妊娠体质量与娠妊结局的关系。方法将剖宫产及经阴道分娩的孕妇按孕周分为12个组进行对照,比较其在各孕周的BMI有无差别;经阴道分娩产妇中有产道撕伤与无撕伤者的BMI进行比较看有无差别;有产道撕伤者与无撕伤者按孕周分为12个组进行对照,比较其在各孕周的BMI有无差别;经阴道分娩产妇有阴道助产与自然分娩者进行BMI比较,研究两组有无差别;经阴道分娩产妇中行阴道助产与自然分娩者按孕周分为12个组进行对照,比较其在各孕周的BMI有无差别。结果妊娠足月时行剖宫产者各孕周BMI均大于经阴道分娩者（P〈0.05）。经阴道分娩有产道撕伤者BMI大于无撕伤者,而两组各孕周BMI比较结果显示：在妊娠32-38周,有产道撕伤者BMI明显大于无撕伤者（P〈0.05）。经阴道分娩者产时行阴道助产与未行助产者BMI及两组中孕周BMI比较显示产时是否行阴道助产与孕期BMI无关（P〉0.05）。结论妊娠期特别是32-37周妊娠应加强体质量监控。高体质量孕妇应适当减少能量摄入,增加能量消耗。妊娠前体质量正常及妊娠期体质量增加正常,不仅可获得良好的妊娠过程和妊娠结局,而且对其远期健康及再次妊娠都是有益的。

Objective To investigate the relationship between maternal Body Mass Index （BMI） and pregnancy outcome. Methods Both cesarean section and vagina delivery women are divided into 12 groups. The BMI is compared between gestational weeks; the BMI between vagina delivery women with or without obstetric canal tear are compared, vagina delivery women with or without obstetric canal tear are divided into 12 groups gestational weeks, the BMI is compared;the BMI between vagina delivery women with or without obstetric aid are compared, vagina delivery women with or without obstetric aid are divided into 12 groups by gestational weeks, the BMI is compared. Results The BMI of full pregnancy women with cesarean section is higher than vagina delivery women in all gestational weeks （ P 〈 0. 05 ）. The BMI of women with obstetric canal tear is high- er than vagina delivery women, the result of different gestational week shows that BMI of women with obstetric canal tear is significantly higher than vagina delivery women in 32-28 weeks （P 〈 0. 05 ）. The BMI comparison between vagina delivery women with or without obstetric aid and in different gestational weeks shows no relation-ship between BMI and obstetric aid （P〉0.05）. Conclusion The BMI inspection of gestational women espe-cially in 32-37 weeks should be emphasized. Pregnant women with high BMI should reduce energy intake and increase consumption. Normal BMI in pre-pregnaney and pregnancy not only can result in good pregnant outcome, but also lead to a good next pregnancy and long term health.
