高血压脑出血术后肺部感染的预防和治疗

Prevention and treatment of lung infection after operation for hypertensive cerebral haemorrhage:A report of 210 cases

目的:探讨高血压脑出血术后肺部感染的预防措施和治疗方法。方法:回顾性分析210例高血压脑出血术后肺部感染的病例,在治疗原发病基础上,采用抗炎、化痰、吸氧、雾化吸入等对症处理,同时应用气管切开71例,机械通气53例。结果:本组病例中治愈142例,死亡68例,其中行气管切开治愈58例,机械通气治愈9例。结论:预防和治疗的重点是清除颅内血肿,合理选用抗生素,监测病原体,减少医源性感染危险因素,加强呼吸和营养支持等,均有利于肺部感染预防和治疗。

Objective ： To investigate the prevention and treatment of lung infection after operation for hypertensive cerebral haemorrhage. Methods：Two hundred and ten cases of lung infection after operation for hypertensive cerebral haemorrhage were retrospectively reviewed,Based on the management of primary injury,antiinflammatory,phlegm dissipation,oxygen take--up and atomization inhalation were adopted, and at the same time 71 were treated with tracheotomy,and 53 were treated with mechanical ventilation. Results：In this series 142 cases got the expected effect,of whom 58 were treated with tracheotomy, 9 were treated with mechanical ventilation, and 68 died. Conclusion：The key to the prevention and treatment is to clear intracranial hematoma, rational use of antibiotics ,surveillance of pathogens, reduction of risk--factors, respiratory and nutrition support ,etc.
