2254份血液菌株培养结果与耐药性分析

摘要 目的了解本院血液感染的细菌种类及耐药性。方法采用全自动血培养仪BACTEC9050进行培养,细菌鉴定采用ATB Expression系统,药敏试验采用K-B法。结果血液培养阳性率为10.16%,检出细菌23种共229株,其中革兰阳性细菌139株(60.70%),革兰阴性细菌76株(33.19%),真菌14株(6.11%)。革兰阳性细菌以表皮葡萄球菌(81/139)为主,革兰阴性菌中以大肠埃希菌(45/76)为主。表皮葡萄球菌对利福平、呋喃妥因、米诺环素、万古霉素和替考拉宁有较高敏感性。大肠埃希菌对哌拉西林/他唑巴坦、头孢吡肟、阿米卡星、美罗培南和亚胺培南有较高敏感性。结论本院血液标本细菌检出率较高,了解血液感染细菌的分布特征和耐药性,对减少院内感染,指导临床合理使用抗生素有重要意义。 Objective To investigate the kinds of bacteria and their drug resistance in blood cultures of our hospital. Methods Bacteria was detected with BACTEC - 9050 Autoanalyzer and ATB Expression. The K-B method was applied for the drug susceptibility test. Results The positive rate of blood cultures was 10.16% and 229 bacterial strains were detected. There were 139 strains of Grain-positive bacteria (60.70%) ,76 strains of Gram - negative bacteria (33.19%) and 14 strain of fungns(6.11% ). The main bacterial in Gram - positive bacteria was Staphylococcus epidermidis and in Gram-negative bacteria was Escherichia coll. Staphylococcus epidermidis were highly sensitive to rifampicin,nitrofurantoin,minocyeline, vaneomyein and teieoplanin, while Escherichia coli to piperacillin/tazobactam, cefepime, amikacin, imipenem and meropenem. Conclusion The isolated rote in blood culture was high and it was of great significance to decrease nosocomial infections and clinical chemotherapy by blood cultures.
