摘要
目的研究急性脑梗死(ACI)患者单个核细胞(MNCs)凋亡的变化,检测周围血中白细胞介素(IL)-1β、IL-8和可溶性细胞间黏附分子-1(sICAM-1)水平的变化,以探讨细胞因子和黏附分子对MNCs凋亡的影响。方法采用流式细胞术检测64例ACI患者和30名健康对照者MNCs的凋亡情况,采用酶联免疫吸附法(ELISA)检测细胞因子和黏附分子水平。结果ACI患者MNCs凋亡率明显高于健康对照组,呈逐渐升高趋势;ACI患者发病72 h血清IL-1β、IL-8和sICAM-1水平明显高于健康对照组(P<0.01),发病第1周达到最高值,至第14天呈逐渐下降趋势,且与MNCs凋亡率均呈正相关。结论ACI患者MNCs凋亡率增加,可能与炎性细胞因子产生过多、免疫细胞黏附分子表达上调密切相关,通过适度调控细胞因子的分泌和MNCs的凋亡可能会改善ACI预后。
Objective To observe the changes of mononuclear cells (MNCs) apoptosis and the level of inflammatory interleukin-1β (IL-1β), interleukin-8 (IL-8) and soluble intercellular adhesion molecule-1 (slCAM- 1) in patients with acute cerebral infarction (ACI), to study the relationship between them in order to explore the effect of cytokines and adhesion molecules on apoptosis of MNCs. Methods The apoptosis of MNCs in 64 cases patients with ACI and 30 healthy controls were detected with flow cytometry. The inflammatory cytokines and the adhesion molecules were tested with enzyme-linked immunosorbent assay (ELISA) method. Results The percentage of MNCs apoptosis in the patients with ACI was significantly higher than those in the healthy control group and the gradually increasing trend was showed. The levels of IL-1β, IL-8 and sICAM-1 in patients with ACI were significantly higher within 72 hours of onset than those in the healthy control group (P 〈 0.01) and at the peak value on 7 days, then gradually declined on the 14th days. The rate of apoptosis and MNCs were positively correlated. The increased levels of inflammatory cytokines and adhesion molecules in ACI patients were positively correlated with the percentage of MNCs apoptosis. Conclusions The high expression of inflammatory cytokines and adhesion molecules might play an important role in promoting MNCs apoptosis which reduced the functional life span of MNCs. For this reason, modulating the cytokine secretion and apoptosis of MNCs might improve prognosis in patients with ACI.
